Suprapubic catheter replacement

Preparation
- catheter kit + catheter
- Chux
- sharpie pen
- non-sterile gloves (2 sets)
- towels (2-3)
- gauze
- tapes
- urine specimen cup if taking urine culture
key points
Make sure this is not the first replacement. The first replacement must be done by physician.
- Wash your hands.
- 1st pair of non-sterile gloves is for marking the tubing with a pen.
- open sterile kit prior to old catheter kit removal to reduce interval time.
- open outer package of the tubing and place an sterile inner package onto the sterile field.
- 2nd pair non-sterile gloves is for removing current (old/existing) catheter.
- Don sterile gloves not on/over the sterile area.
Documentation
- Physician/Urologist order for replacement
- Consent (patient/facility/ )
- Date of the procedure
- Clinician name and title
- Positioning (supine is preferred)
- Patient condition (no distress?)
- Assessment of the insertion site (skin, tract integrity)
- Old (removed) tubing type, size, balloon size
- New (inserted) tubing type, size, balloon size
- Urine output (immediate? Clear yellow?)
- Securement device of the new catheter
- Patient’s tolerance (tolerated well?)
- Any complication (pain, bleeding, discomfort, difficulty of insertion)
- Connected to the drainage bag?
- Urine specimen order? collected?
